 2003 Walk to D'Feet Walk Team Captain Sign-up Form

On October 4, 2003, the ALS Association of Greater Los Angeles will host the 3rd Annual Walk to D'Feet ALS fundraiser.  Last year's event raised over $250,000 for the chapter.  This year's event promises to be the biggest event ever!  Thank you all for your support, donations and participation!
